Both situations are identical. Without the availability of a legal means of obtaining what one wants the alternative is doing it illegally. Being illegal isn't going to stop anyone, it's just going to add unnecessary risk and jeopardize individual freedom.

There are plenty of cases of gender affirming care that do not involve transitioning to the opposite sex. Men who lost testicles due to torsion or physical damage, women with double mastectomies, women with alopecia, men with gynecomastia, etc.
