You mentioned you forgot to push your chest up prior, you buried your chin into your chest during the walkout your chest will always follow the head, start by keeping your head up and your chest should follow. That weight was pretty light for you as you get closer to your max if you don't get your head/chest upper back into position you will most likely dump the weight forward. I would get set prior to my walkout so the only thing that changes is your foot position.
