Exactly where is the pain? Exactly what motion do you need to do for exaggerated pain, and which part of that motion?

Sounds to me like the humerus is not being kept centrated in the glenoid fossa (the socket that is), and it will cause pain, tightness and inflammation, usually.
Another important aspect to consider is that the scapula might not be functioning properly, as it should rotate posteriorly and abduct (brace around the thorax) as you OHP.

The OHP is usually the first exercise to hurt, as it's one that needs perfectly healthy shoulders to keep doing. While such as the decline bench, can be done with quite unhealthy shoulders (which often perpetuates the condition)
