A friend of mine is about 100lbs overweight. He’s gonna start Reta. Can you guys give some guidance on a good starting dose and frequency?

thanks!

I would start at .5mg x 2 a week (Monday/thursday)

then .75mg x 2 a week after 2-3 weeks.
Repeat this titration pattern as needed to arrive at a dose that is effective for reasonable weight loss without side effects. Somewhere between 2 mg and 8mg is where most people end up

if they experience side effects, back off the dose nominally for the next week-2 weeks to manage the side effects.

I'd pick tirzepatide over reta for massive weight loss. It just nukes your hunger better.

This is true if not being about to pass a plate of food without eating it is the problem. However, if food discipline is okay then reta should be enough to suppress the appetite enough to make smart choices and its effects on the metabolism are far greater than tirzepatide.

Run with the minimum effective dose and be mindful of duration being used. I've heard of people having a lot of GI issues down the road.

Also, when they come off, be mindful of the rebound. One of the negatives I see for GLP's are that the root issue of why your 100 pounds overweight doesn't get addressed. Some of the mental aspect is bypassed and not dealt with. I would not recommend using a GLP into perpetuity so honestly some therapy or counseling during your friend's journey could be big help.

i have a friend on it, I actually recommended it because it it's a three prong attack. Reduces hunger, increase energy expenditure and better glucose metabolism. Start slow 0.5mg/wk for 2 weeks and 1.0mg on week 3 and 4. Assess and go from there. The party really doesn't start until you are at 1.0 mg but those first 2 weeks lets you body get use to the drug.

Most have little to no problems with 2.0 mg.

I've read it reduces cravings of all kinds....food, alchohol, tobacco and drugs. Cuts out the "noise".
It’s true. Even makes it hard to get drunk. A couple times I had a single beer and I didn’t want a second and hardly could finish the first or wanted to. Major craving reduction no matter what you were craving. One thing that is a factor is you can get some anhedonia because the dopamine response from things that otherwise would be pleasurable are noticeably dulled.

Good advice overall. Start with 0.5 mg twice a week and remember it takes 4 - 6 to get to steady state. Ramp up slowly and only if needed. RETA is superior for metabolism but not as effective for appetite suppression. Two of my clients add a micro dose of SEMA to their RETA (2 mg RETA and 0.25 mg SEMA total a week), that gets them just enough appetite suppression wo GI sides and metabolic stim as well. A small amount of GH (1 -2 IU) and HRT T dosing is a perfect mix for a solid overall effect and low risk.

Good info here I’ll use this method.
