Raw I only wear a belt for my heaviest sets..... depending on how I feel.

In gear I use it for everything.

The big thing is that most people misuse a belt. They try to cinch it down and think it'll support their back.
That won't help much. A belt doesn't help the back muscles as much as it does the abs. Get the belt snug and just push your belly out in to the belt. That bracing out in to the belt is what provides most of your support.

I usually put on a belt when I'm on my second to last warm-up before my working sets. It's not just the % off of 1RM, but the number of reps/sets plays a factor as well.

If I'm doing 300x3 x5-6 sets, then my warm-up at 225 will likely be without a belt.
If I'm doing 225x8 x4-5 sets, then I'll be wearing a belt.
