Hey TID whats good? Hoping everyone is doing well and getting it in @ the gym. Well , Ive come to a decision. My doctors suggests to go in Active Surveillance. And for now I like the sounds of that. In six months I go back and see him and run some tests. If there are any changes we can do what is necessary. So now that wifey and I agreed to do the AS , I am @ an all time low , I feel like crap , I havent been to gym , there is no motivation to workout. Wifey doesnt want me to get back on cycle period. She says that I shouldnt take the risks. I feel differently though. A small Test E cycle for a few months couldnt hurt right ? Or could it ? I am asking this to all those bros that have been in my shoes or better yet have tried on some other shoes that didnt fit :) Maybe its too soon or maybe I should just chill and not worry about my weight which again is 149.2 is the lowest Ive been in decades. I feel good at 175lbs -180lbs. My bedroom skills have plummeted. I have no interest and its bothering me . Dr suggests maybe Cialis . I feel that test will bring me back up to my normal weight and I could get back into the swing of things in the bedroom :) What say Ye ?

I believe there are more recent papers on the subject. T does not seem to influence progression, do a lit search for Abraham Morgentaler, he's an expert in the area. In my non-medical opinion, I'd be more concerned with low vs high T with PCa.

This article gives a lay version of T and PCa. It relates to bipolar treatment, but the takeaway from this, at least mine is, 1) low to normal T may be the sweet spot for PCa progression. High and very low seem to be inhibitory, 2) T doesn't seem to increase the progression of metastatic and give QOL. Certainly lots of questions that still need to be answered and much more research to be done. PCa like everything else is very complicated and individualized, so many genetic variations that dictate responses to treatments. Thanks for that W6 ! Ive never done TRT before, my thing was 4 months on 4 off. I would like to get some of my weight back and my willy doing the wonka if you know what I mean :) I haven't shared with my Dr. anything that has to do with AAS . So I dont know if I should ask him to place me on TRT. I do have some left over that I can easily break down and do my own TRT. SO here goes, what does trt look like ? I mean it is a scheduled thing right??

This is something that needs to be closely monitored by a doc that works with guys with Gleason 6 PCa and HRT. First you have to know what your baseline T, fT, E2 and SHBG are and then proceed from there if you are even low. Then PSA probably at 3 month intervals at least until you get a handle on how you respond. Talk to your doc about it.

Then PSA probably at 3 month intervals at least until you get a handle on how you respond. Talk to your doc about it.
Ok so for now I will chill. Wait until my 6 month visit comes up . I will bring it up and take from there just like you suggested. OK , that was fairly easy :) Knowing me though I wont wait the 6 months and email his ass pronto to see if he would consider checking me out to see if Im even eligible . Thanks Wilson. Really . Just talked with my soon to be 81 yr old friend that has a similar grade PCa, PIN, etc.. His PSA 6 mo ago was 6.2 and this week 6.1. He's been on a mix of ND and TC 200 mg/10 days for decades and his docs told him to continue, it made no difference. Just an FYI. His ECHO and cardiac function is also fine. VO2 that of a fit 50 yr old. Joints are trashed, but who's are not at that age, or our age.

Wow. Where does one find doctors like your 80yo friend has ... doctors who are not rigidly committed to same old - same old "care-giving"? Are doctors like this usually at an age where they too are experiencing, or at least worried about, their own "male problems"?

Does his ND/TC mix come from a script, or is he doing DIY and they are just aware of it?

He was a rare find. Even when he started his practice at a major institution he was prescribing 200 mg/wk of TC, had the largest number of guys on HRT at that institution, even some of the docs. Best patient satisfaction rating of the entire urology dept and it was a big dept. The endo and internists were critical of the dosing and even HRT but there were no problems associated with it. Yes he is having a number of age related issues, used to be bullet-proof in the 80's with his sports cars and chrome suits, but father time has a way of making at all hurt when you get old and humbling. Brilliant doctor and surgeon, but also open minded. The TC and ND is script, there are several compounding pharms that make ND. Most of the docs that use it in HRT a 2:1 ratio TC/ND, maybe 1:1. No one makes a blend to my knowledge, just mix your own based on needs.

UPDATE.

Hello again TID family. So heres the latest. So Ive got some crazy results that the dr has no clue how this is , But first let me explain that Im currently on Active Surveillance until a test comes back and says that my cancer has grown. So the last PSA was very low not like the one that threw up the flags. Check it

so in November when the initial PSA was done my PSA was an 11.10 . Now the last one I got done was on July 12th and that was a 0.60 . My doc was like maybe you had an infection or something . I will be honest here though I was coming off a 12 week test cycle maybe thats why it was high? Of course I didnt divulge that to my doctor. I did not mention AAS at all to my doctor. I aint gonna lie , I dont think I will ever admit that to him. But Ive heard of others sharing that with their doctors but again I dont have an Rx for my cycles so theres that . Anyway just wanted to give everyone an update because you all were very very freaking helpful and showed me love and I was going through some issues and you all have helped me in the past . Again I truly want to thank the TID fam for everything . I will be back during the week to check out the replies . Take care , GOD bless you's all.
