New guy, had some lab work for trt and had some concerning surprises.

Basics, male, 48, 30 lbs overweight, never smoked or done any drugs, social drinker, no other health issues or meds. Lift three times a week, 2-3 days of cardio, mostly LISS with some occasional HIIT mixed in. Never eat fast food, never drank soda, avoid seed oils, etc. career as a first responder. Wound up at a clinic due to just feeling a bit run down, horrible insomnia, pretty sure I don’t have sleep apnea but I’m scheduled for a sleep study, no motivation or libido, etc. labs came back where everything was in the middle of the normal range with the following exceptions.

1) total T - 550 range 264-916
2) free t - 6 range 6.8-21.5
3) AST (SGOT) 46 range 0-40
4) CRP (C reactive protein) (Cardiac) 6.24 range 0-3

full labs are attached.

Needless to say that c reactive protein number got my attention. Not really sure what to do about that. Meet with the TRT Dr later this week to go over everything. Not sure what direction he’ll go.

Anyone have any feedback on these numbers or similar experience? Thanks I’m advance for your help.

Could be diet. Could be poor tolerance to oil or solvent in you TRT preparation. Could be a number if things.

Thanks for the reply. im not currently on TRT. The above lab work was what my dr initially ordered to see where I’m at. I assume he’ll offer TRT since my free is so low but there may be other options? Im new to all this so I don’t know if stuff like DHEA will be enough to get my free up. Im sure losing weight will certainly help but how much, I don’t know.

CRP is a marker for inflammation. Probably one of the best things you can do is eat very clean like close to the earth. Nothing processed, moderate protein, moderate to low carb, and low to moderate fat adjusting accoring to activities and your average burn rate. Track it to dial it in. Produce isle, meat isle and clean carbs like basmati rice, potato etc. There is so much inflammatory shit in our food supply now it's horrendous.

I had an MRT panel done. It's not like the IgG or IgE tests offered by mainstream services like Everlywell or whatever. It was very informative and shows actual sensitivities. Since I dropped all foods to which I was shown to be sensitive, I have zero bloat and gas. Inflammation in general is way, way down. I used to not even be able to get up off the couch without being hunched over a few seconds due to my back. Crouching down to pick something up was a real effort. And I had been just living like that for a while, used to and resigned to it. Corn is the absolute worst offender for me, and it is in so many places and goes by different names as an ingredient. The crazy thing is I'm sensitive to just about every alternate grain available, even stuff I've never eaten.

TL;DR. Eating clean and avoiding your sensitivities is real.

I do agree about the crap in our food these days. What frustrates me is that I don’t eat bad. Never soda, very rarely eat out, don’t drink juice, eat candy bars, etc and yet I gained weight. I eat too much and am tracking my food now, weighing everything.
I’ll be curious to see what this dr says about my low t and if trt will help or hurt this inflammation. Thanks for the reply.

Man that sounds like a rough road. I don’t have any sensitivities like yours but I do have an odd one. When I avoid bread, grains etc (except oatmeal and homemade sourdough) and then have a tortilla, hamburger bun or something, within a few hours my knees ache. I’ve tested that theory a few times and like clockwork, within a few hours, my knees feel 20 years older. I agree with you that food sensitivities are real.

When I avoid bread, grains etc (except oatmeal and homemade sourdough) and then have a tortilla, hamburger bun or something, within a few hours my knees ache. I’ve tested that theory a few times and like clockwork, within a few hours, my knees feel 20 years older. I agree with you that food sensitivities are real.
Well around your age my test was still mid ragne but I was in the mid-life decline, gaining weight etc. I just went on and went back to cycling like I had a decade before that. It worked and it still works. Stick with it. It takes a while eating clean and tracking things but eventually, when you get your anabolism and training right things will start popping again.

Wow, C reactive protein is through the roof! What did the doc tell you about the neutrophils? How is you immune system?

Sleep apnea can increase CRP and make you feel like shit. Start with the sleep study to rule out OSA. AST of 46 is fine. LDL is high. When was your last bout of exercise relative to the labs? Exercise can cause an acute increase in CRP, nothing in the CBC suggests acute inflammation. ALT is likely elevated from exercise. If you lift it is always good to check CK along with the labs, if elevated several fold, ALT, AST and LDH will also be elevated from muscle damage. A calcium CT scan would be a good idea. If you want good baseline data, take a week off from exercise, just walking and then get labs done see if that brings down AST and CRP. In summary, I would recheck labs after a week off for a good baseline, get a coronary CT score done and the sleep study. Start there see what comes of it.
