First thing they do is scan your drivers license and then your insurance card. I’m not sure what would happen if you said you didn’t have an ID. Like GF said the names have to match in my state too. I would guess they would turn you away until you provide something that meets their requirements.

Yeah, the LabCorps I go to actually takes your ID from you after you scan it to verify. The Quest I use just has you scan it to check in then verbally confirms your information when you go back, but the ID still needs to match the information or it won't even check you in. I have sat there and watched people with maiden names and suffixes get hung up trying to check in and they would have to take them back and manually check them in.
